- When requesting an application guide, be sure to enclose a self-addressed envelope (A4 size) with a 390 yen stamp affixed on it, and also enclose a memo specifying the program and course (choose one out of Master's Program, Doctoral Program, Three-year Doctoral Programs, or Medical Science Program) and the requestor's contact address on it.
