How to request

You can either bring or post the requisition form. When requesting by mail, be sure to enclose an requisition form filled out necessary information and a self-addressed envelope. (Requests by telephone or e-mail will not be accepted.)
Certifications are issued free of charge.

Academic service offices on Tsukuba Campus: 9:00-12:15, 13:15-17:00 on weekdays
Academic Service Office for the Business Sciences Area on Tokyo Campus: Monday: 10:00-18:30
Tuesday-Friday: 10:00-21:10
Saturday: 11:40-20:00

1. Requests for certificates
Please request after filling out the requisition form. The form is downloadable from the following sites.

2. Return-mail envelope
Be sure to enclose a stamped and self-address envelope (120 x 235mm).

- A stamp for regular mail: A 80 yen stamp for less than three certificates, A 90 yen stamp for less than seven certificates
- A stamp for "Confidential" mail: A 80 yen stamp for less two certificates, A 90 yen stamp for less than five certificates

Remarks

In case you request for a certificate other than the university specified form, please contact the respective office in advance.
Undergraduate credited auditors and graduate credited auditors may request at respective academic service offices.
Certificates are issued at respective academic offices. Therefore when requesting more than one certificates, please request at respective offices.
