Request for Brochure
To request for an extension course brochure, please send a mail to the above office. For courses held in Shimoda and Sugadaira, please send the request to Tsukuba Office.
The brochures will be available from the end of February.
How to apply
When requesting a course brochure, be sure to enclose a self-addressed envelope (A4 size) with a 140 yen stamp affixed on it and mark "Request for 2013 Extension Course Brochure" on the outside of the envelope.
Application
Applications are accepted 2 - 3 months before the course starts. Application deadline of the scheduled extension course will be posted on the website. Click on the course name to download the details and application form of the course.
Note
For certain courses, the participants may be responsible for arranging their own materials, material cost, supplies expense, travel expenses, and accommodation fee in addition to the course fee.

2. Applying by mail or bringing the application form to the office
You can mail or bring the application form to the office in charge stated left ("Offices for Application/Inquiries").
Be sure to enclose or bring a self-addressed envelope (12x23.5cm) with a 90 yen stamp affixed on it when you apply.

Be sure to use designated forms when you apply as the form varies from course to course. - Course name
- Full name (with hiragana beside each kanji), sex, age, and the date of birth
- Zip code, address, and telephone number
- Place of employment and its telephone number, or name of educational institute and grade
- Whether or not you wish to avail the "Sports Anzen Hoken" as recommended by the university.
- Those who do not intend to avail the above insurance should specify the name of insurance covering them.
- Those applying for the golf course (beginners' level), please specify when you started playing golf.
- Those applying for the golf course (advanced level), please specify the time when you took the intermediate level course.
* To apply for general education, sports, and art courses, the following infomration written on the A4-size paper are required to submit.
To apply for sports course, following information is required to provide.
Acceptance
In case the number of applicants exceeds the scheduled number of applications, successful applicants will be decided either by lot or on a first come first served bases. Results of screening and detailed information will notified to all applicants.
Course fee
Please note that course fee once paid shall not be returned.
Sports safety insurance or accident insurance
Participants who will enroll in courses that include sports lessons, outdoor practice or trial runs are required to take the sports safety insurance or accident insurance.
Certificate of completion
Those with an attendance level of over seventy percent will be awarded a certificate of completion.
Certification of course completion
Those who wish to receive a certification of course credits completed are requested to download the following form, place it in a self-addressed envelope (A4 size) with a 200 yen stamp affixed on it and post it to the following address.
