Guide to Workplace COVID-19 Vaccination (University Vaccination)

To reduce the burden of COVID-19 vaccination on other areas and speed up vaccination, the University of Tsukuba will implement workplace COVID-19 vaccination at the university (university vaccination).


【Important Notice】

  • If you have already received your first dose of vaccine at a large-scale vaccination center etc. outside the university, you will not be able to make an appointment for your first dose at the university. However, if you have any unavoidable reason, please consult with us.

1. Persons Eligible for Vaccination

○ Students in the University of Tsukuba (Including credited auditors and non-degree research students)

○ Faculty members and employees in the University of Tsukuba (Including visiting faculty members and lecturers (non-full-time)

○ Others (Security, cleaning, temp staff, etc. who perform tasks on a routine basis in school)
* In principle, those who can complete 2 doses of vaccine at out university.



【Notes】

Vaccination is by reservation only. Please note that walk-ins will not get vaccinated. (For the reservation method, refer to "4. Reservation method".)

○ For the Moderna COVID-19 vaccine, 2 doses are required.

○ Because there are those who cannot get vaccinated for reasons of constitutional predisposition, pre-existing condition, etc. or risks for side effects, getting a vaccination is non-compulsory. Furthermore, please be considerate of those who do not wish to get vaccinated for them not to be given discriminatory treatment.

○ This vaccine cannot be administered to people for which the following apply. If you think you correspond to one of the following, be sure to tell the physician during the prevaccination consultation if you think any of the following apply for you.


  • People with obvious fever (*1)
  • People suffering from serious acute illness
  • People with a history of severe hypersensitivity (*2) to any of the ingredients of this vaccine
  • People other than those described above who have conditions that make it inappropriate for them to receive the vaccine

  • (*1) Obvious fever is usually defined as 37.5℃ or higher. However, this does not necessarily apply to cases in which it is judged as a fever considering the normal body temperature, even if the temperature is below 37.5℃.
  • (*2) Anaphylaxis and multiple symptoms suggestive of anaphylaxis, including generalized skin and mucous membrane symptoms, wheezing, dyspnea, tachycardia, and hypotension

○ For the vaccination, those who demand special attention, establish preliminary contact with the secretariat.




2. Vaccination Period

(1) From Monday, August 30 to Wednesday, September 22
(Excluding Saturdays, Sundays, national holidays, September 7 and 8)

(2) From Monday, September 27 to Wednesday, October 20
(Excluding Saturdays, Sundays, October 5, 6 and 18)


Vaccination Hours: from 13:00 to 17:00


* The vaccination schedule may change according to the supply schedule of the vaccine, etc.




3. Vaccination Site

* The location of the reception is the foyer in front of the 2nd floor Auditorium. Vaccination Site

* For the parking lot, use the Art and Sport Sciences West Gate Parking Lot. (The gate is open during the period.)




4. Reservation Method

For the reservation method, reservations can only be made at the reservation site. Reservation cannot be made by phone.

【Students】
  • To avoid error when multiple users access the reservation site at the same time, the scheduled email delivery date and time (reservation start date and time) is set by the following affiliated organization. The URL of the reservation site will be delivered to the email addresses in the Zengaku (university-wide) Computer System or email addresses registered in the University of Tsukuba Safety Confirmation System (ANPIC)initial registration site at the following date and time so get a reservation after checking the above entry method.
  • Students will receive an email from the University of Tsukuba Safety Confirmation System (no-reply@jecc.jp).

  • If you do not receive the email even after 1 day from the reservation start date and time, send an email to the email address in the contact information at the bottom. In doing so, clearly state your affiliation, name, and student ID number.

【Notes】

Because of the need to determine the quantity of the vaccine, complete the reservation by the day before the preferred date of vaccination. Reservation cannot be made on the day of vaccination.

For the reservation of the 2nd dose, obtain the reservation on the reservation site by yourself after obtaining the reservation of the 1st dose.

○The 2nd dose of the vaccination is assumed to be received after 4 weeks (28 days) from the 1st dose of the vaccination.
(The reservation period of the 2nd dose will be displayed on the reservation site.)

○ Be sure to register the reservation of the 2nd dose. If you forget to reserve, you cannot get vaccinated.

○ When changing the reservation date and time, reserve and register once again after canceling.

Cancellation of reservation must be made by the day before the reservation date. Cancellations should be made from My Page of the reservation site

○ After registering the reservations of the 1st dose and 2nd dose, if you cancel the reservation of the 1st dose, all reservations will be canceled.

○ The reservation time frame is on a 15-minute basis, however, for a trouble-free vaccination, please come at the start of the reservation time frame. (Example: If the reservation time is from 13:00 to 13:15, come to the reception by 13:00.)




5. Things to bring on the day of the vaccination

(1) Prevaccination Screening Questionnaire for COVID-19 Vaccine(予診票)

  • Refer to "7. Prevaccination Screening Questionnaire for COVID-19 Vaccine(予診票)"

(2) Vaccination Voucher(接種券)

  • Bring only if it has already arrived from the local government.
    (Bring the Vaccination Voucher(接種券) without detaching the stickers on it.)

(3) Record of Vaccination for COVID-19(接種記録書)

  • 1st dose: Print form, fill out the "Vaccination Date", "Name", "Address", and "Date of Birth", and bring it with you.
  • 2nd dose: Fill out the "Vaccination Date" in the Record of Vaccination for COVID-19(接種記録書) which was returned to you during the 1st dose of the vaccine and bring it with you.

(4) ID (Student ID card, employee ID card, driver's license, etc.)

  • If your identity cannot be confirmed, please note that you cannot get vaccinated.

 When the reservation is fixed, print out the Prevaccination Screening Questionnaire for COVID-19 Vaccine(予診票) and Record of Vaccination for COVID-19(接種記録書) from the following by the day of the vaccination, fill out the necessary information, and remember to bring it on the day of the vaccination.


【Notes】

○The vaccination requires the Vaccination Voucher(接種券) distributed by the local government. In the case that you have not received it from the local government, you can be vaccinated without the "Vaccination Voucher(接種券)", however, submit it after the 2nd dose of the vaccine and be careful not to lose it.

○ For the 2nd dose of the vaccine, in the case that you have not received the "Vaccination Voucher(接種券)", as soon as you receive the "Vaccination Voucher(接種券)" after the 2nd dose of the vaccine, students should submit it to the Student Affairs in the respective Academic Service Office, faculty members and employees to the General Affairs of the affiliated department, and the rest to the corresponding department.




6. At the site on the day of the vaccination

(4) Issuance of the Record of Vaccination for COVID-19(接種記録書) and collection of the Prevaccination Screening Questionnaire for COVID-19 Vaccine(予診票) (3rd floor Hall)

(5) Standby (3rd floor Hall) from 15 minutes to 30 minutes

(6) In the absence of side effects, this is the end of the vaccination (Exit from the 3rd floor Emergency Staircase)


【Notes】

○ For a trouble-free vaccination, please dress in something easier for you to bare your shoulder such as a shirt with short-sleeves. Furthermore, please wear a mask.

○ We cannot lend a change of clothes from the standpoint of infection countermeasures.




7. Prevaccination Screening Questionnaire for COVID-19 Vaccine(予診票)

Please fill in the bold frame and be sure to bring it on the day of the vaccination.
Check the "Address on the resident card" and fill in the address.

○ Read the "Instructions for the COVID-19 vaccine" and get the vaccine after understanding the effects, adverse reactions, etc.。

○ For the "Body temperature before examination", check your body temperature before coming to the vaccination site. For those who cannot check their body temperature in advance, it is also possible to check your body temperature at the site.




8. What to do after receiving the vaccine

○ After receiving the vaccine, please wait at the vaccination site (3rd floor Hall) for at least 15 minutes (at least 30 minutes for those who have experienced severe allergic symptoms including anaphylaxis in the past, or those who have felt sick or fainted, etc.), and if you feel unwell, please contact the staff immediately. (This makes it possible to respond to sudden adverse reaction.)

○ The injected area should be kept clean. Although bathing on the day of vaccination is not a problem, please do not rub the injected area.

○ You can usually return to you normal daily activities, however, please refrain from strenuous exercise and avoid excessive alcohol intake on the day of the procedure.




9. Side Effects

○ The main side effects include pain in the injected area (*1), headache, joint and muscle pain, tiredness, chills, and fever. Rare and serious side effects include shock or anaphylaxis. Because this vaccine is a new type of vaccine, there is a possibility that it may cause symptoms that have not been clarified so far. If you notice any symptoms after vaccination, consult your family doctor.

(*1)Many people feel pain in the injected area the next day than immediately after the vaccination. Pain, swelling, etc. may occur one week after the vaccination.

○ While rare, there is a risk of mild myocarditis and pericarditis after the vaccination (*2). If there are symptoms including chest pain, palpitation, breathlessness, edema, etc. within a few days after vaccination, get immediate medical attention.

(*2) Confirmed cases have occurred mostly in young males more often after getting the second dose than after the first dose.
